  Violation: French fry cutter, bin for utensils, and refrigeration units and seals are needing to be cleaned. The lines for the
  pop syrups are sticky and need to be cleaned to sight and touch.

  (A)Equipment, food-contact surfaces, nonfood-contact surfaces,  and utensils.
      (1) Equipment food-contact surfaces and utensils shall be  clean to sight and touch.
      (2) The food-contact surfaces of cooking equipment and pans  shall be kept free of encrusted grease deposits
      and other soil accumulations.
      (3) Nonfood-contact surfaces of equipment shall be kept free  of an accumulation of dust, dirt, food residue,
      and other debris.
   
  3717-1-04.5 Equipment, utensils, and linens: cleaning of  equipment and utensils.
  (D)Nonfood-contact surfaces - cleaning frequency.
  Nonfood-contact surfaces of equipment shall be cleaned at a  frequency necessary to preclude accumulation of soil

  Violation: Sink in employee bathroom has a loose faucet and needs to be repaired. Also, there is a steady leak and puddle
  under the floor in the ladies room in the seating area. Plumbing needs to be maintained in proper working order.
   
  3717-1-05.1 Water, plumbing, and waste: plumbing system.
  (S)Plumbing system - maintained in good repair.
  A plumbing system shall be:
      (1) Repaired according to the Ohio building code; and
      (2) Maintained in good repair.

  Violation: Missing tiles in the tap room floor and the flooring in the kitchen is damaged and needs to be restored. These
  areas must be maintained smooth, durable and easily cleanable.
   
  3717-1-06 Physical facilities: materials for construction and  repair.
  (A)Indoor areas - surface characteristics
      (1) Except as specified in paragraph (A)(2) of this rule,  materials for indoor floor, wall, and ceiling surfaces
      under conditions of normal use shall be:
          (a) Smooth, durable, and easily cleanable for areas where food service operation or retail food establishment
          activities are conducted;
          (b) Closely woven and easily cleanable carpet for  carpeted areas; and
          (c) Nonabsorbent for areas subject to moisture such as  food preparation areas, walk-in refrigerators,
          warewashing areas, toilet rooms, mobile food service  operation or mobile retail food establishment servicing
          areas, and areas subject to flushing or spray cleaning  methods.
   
   
  3717-1-06.4 Physical facilities: maintenance and operation.
  (A)Repairing.
  The physical facilities shall be maintained in good repair.

  Violation: Stained ceiling tiles in the tap room need to be  replaced or painted after any water leaks are addressed.
   
  3717-1-06.1 Physical facilities: design, construction, and  installation.
  (A)Floors, walls, and ceilings.
  Except as specified under paragraph (D) of this rule and except  for antislip floor coverings or applications that
  may be used for safety reasons, floors, floor coverings, walls,  wall coverings, and ceilings shall be designed,
  constructed, and installed so they are smooth and easily  cleanable.

  Violation: Light bulbs in the food storage areas must be shielded or shatterproof. Light bulbs in the two walk in coolers does
  not meet that definition at present. LED lights, in general, are shatterproof by design and are bright enough to properly
  illuminate the walk in cooler (low light intensity at present).
   
  3717-1-06.1 Physical facilities: design, construction, and  installation.
  (I)Light bulbs - protective shielding.
      (1) Light bulbs shall be shielded, coated, or otherwise  shatter-resistant in areas where there is exposed food;
      clean equipment, utensils, or linens; or unwrapped  single-service or single-use articles. This paragraph does
      not apply in areas used only for storing food in unopened  packages, if the integrity of the packages can not be
      affected by broken glass falling onto them; and the packages  are capable of being cleaned of debris from broken
      bulbs before the packages are opened.
      (2) An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending
      beyond the bulb so that only the face of the bulb is exposed.

  Noted that there is currently pest control being done by Orkin for roaches. Maintaining the kitchen areas clean, food and
  dirty rags covered tightly, grease residues and sticky residues from alcohol and soda will be very important in pest
  management practices. There are also mouse traps located throughout. Part of controlling mice is making sure doors are
  tightly sealing, this can be achieved using drag strips and weather proofing if needed.
   
  Source of oil dripping down the outside wall of the walk in cooler should be investigated to prevent potential equipment
  failure.

  The vomit and diarrheal cleanup kit must contain a disinfectant that is effective against norovirus. The current disinfectant
  is not. You can obtain a commercially available kit, and that coupled with directions for use and employee training will meet
   the requirement for this rule.
